Referral

在oncall期间，收到无数的Referral。

当了MO call后，才体会之前学长学姐说的“令人抓狂的Referral”。


Referral A

时间：晚上10点
A     : Excuse me, is this Dr Ng? I wish to refer a case.
我   : Yes, please.
A     : I have this gentleman, who fell down 1 month ago, complaining of having lower back pain. We could not see any fracture from X-ray, but we would like your opinion.
我   : Ok…..What is the urgency of referring late this night? Is he having weakness or pain increasing?
A     : No, because my specialist did round just now and she thinks we need to refer to orthopedic.
我   : ...........

[感想：Refer case要分轻重，三更半夜才call说要refer minor case，人家不气才怪]


Referral B

B    : Dr Ng, I would like to refer you a case，patient had a fall 2 days ago, suddenly today he develops painful swelling of the right leg with blisters. X-ray done previously showed no fracture.
我   : Ok, what’s your impression?
B     : err……swelling right foot?
我    : Yes, what do your boss thinks?
B     : Dunno….but please come have a look, k.
我    : ...................

[感想：Refer case 前自己也该做一些功课，适当的检查，不然就像refer 病人shortness of breath [SOB] 给medical 却没有diagnosis，够无聊吧]

Referral C

C    : Dr Ng, I am xx from Hospital xx, I would like to refer a case with fracture left leg. This is a 60 year-old gentleman……….
我   : Is he keen for operation?
C    : Yes, he is.
我   : Ok, sent the patient over.

两个小时后...........
我   : Pakcik, mengapa tak mau operation?
P     : Saya nak cuba ubat kampang dulu. Tadi dah bagi tau doktor sana, tapi dia kata nak hantar untuk jumpa doktor pakar untuk bincang.
我   : .....................

[感想：如果病人不想动手术，sent过来我又能做什么，只是可怜病人得自己乘车子回到遥远的家]


Referral D

D    : Dr Ng, I’m HO from medical, I would like to refer a case, a 82 y/o lady, known case of DM, blah….blah…..

三分钟后............
我   : What’s the diagnosis?
D    : Bed Sore.
我   : Why didn’t you mention at the first sentence?

[感想：要refer case 也得有些技巧，先告诉对方diagnosis，这样才能专心听下去。不然长篇大论后，谁还记得你在说什么]


在骨科当MO call时，脾气确实因为一些抓狂的referral变差了。
但现在总算可以卸下MO的外套，到实习期的下一站去：小儿科。

希望能在那里建立好的基础，学习更多的知识，好好地服务一群可爱的small people。
